Macule

Flat, < 1cm.
(e.g., Freckle, Measles)

Patch

Flat, > 1cm.
(e.g., Vitiligo, Melasma)

2. Raised Solid Lesions

Papule

Raised, < 1cm.
(e.g., Acne, Wart)

Plaque

Raised, > 1cm (Plateau-like).
(e.g., Psoriasis)

Nodule

Raised, solid, deep component, > 1cm.
(e.g., Cyst, Lipoma)

3. Fluid-Filled Lesions

Vesicle

Clear fluid, < 1cm.
(e.g., Chickenpox, Herpes)

Bulla

Clear fluid, > 1cm.
(e.g., Bullous Pemphigoid)

Pustule

Contains pus (Yellow/White).
(e.g., Folliculitis, Acne)

4. Others

  • Erosion: Loss of epidermis only (heals without scar).
  • Ulcer: Loss of epidermis + dermis (heals with scar).
  • Lichenification: Thickening of skin with exaggerated skin lines (due to scratching).
  • Excoriation: Scratch marks.
  • Crust: Dried serum, blood, or pus (scab).
  • Scale: Flaking of the stratum corneum (e.g., Psoriasis, Tinea, Pityriasis versicolor).
