Standard regimen only. This quick guide is intended for adults where standard colchicine dosing is appropriate. Renal impairment, hepatic impairment, significant drug interactions or existing colchicine prophylaxis may require a different approach.

Colchicine for acute gout

How much colchicine is given at the start of an acute gout flare?

The usual Malaysian regimen starts with 1 mg, followed by 0.5 mg one hour later.

Can another colchicine dose be given immediately after that?

No further tablets should be taken for 12 hours after the initial 1 mg plus 0.5 mg regimen.

What can be given after 12 hours?

If treatment is still necessary, colchicine 0.5 mg may be used every 8 hours until symptoms are relieved, subject to the overall treatment-course limit and individual clinical factors.

What is the maximum colchicine dose for one treatment course?

The course should stop when symptoms resolve or once a total of 6 mg has been taken. Another treatment course should not be started for at least 72 hours.
