Oseltamivir
Renal Dose
Renal-adjusted influenza treatment and post-exposure prevention dosing for adults and adolescents aged 13 years and older.
Select the clinical purpose and renal status. For patients not on dialysis, enter an already established creatinine clearance (CrCl). This tool does not calculate CrCl.

Oseltamivir renal-dose FAQ
What is the oseltamivir treatment dose at CrCl 45 mL/min?
For an adult or adolescent aged 13 years or older with a creatinine clearance above 30 and up to 60 mL/min, the Malaysian product insert recommends 30 mg BD. For a standard uncomplicated treatment course this is given for 5 days.
What is the treatment dose at CrCl 20 mL/min?
At creatinine clearance above 10 and up to 30 mL/min, the recommended treatment dose is 30 mg OD.
What happens when CrCl is 10 mL/min or lower?
The current Malaysian Tamiflu product insert lists oseltamivir as not recommended at CrCl ≤10 mL/min because dosing data are unavailable.
What dose is used in haemodialysis?
For treatment, the product insert gives 30 mg after each haemodialysis session. For prevention, it gives 30 mg after every second haemodialysis session.
What dose is used in CAPD?
For treatment, the product insert gives 30 mg as a single dose. For prevention, it gives 30 mg once weekly. These data are derived from CAPD and should not automatically be applied to APD.
Can I use this renal calculator in a child aged 12 years or younger?
No renal dosing recommendation is available from the current Malaysian Tamiflu product insert for this age group because clinical data are insufficient.
