Acute asthma only: prednisolone doses differ substantially for other conditions such as nephrotic syndrome, autoimmune disease, croup and haematological disorders.
Clinical assessment remains essential. Systemic corticosteroids should be used when indicated by the severity and clinical course of the asthma exacerbation. Escalate patients with respiratory distress, hypoxia, exhaustion, silent chest, altered consciousness or poor response to initial bronchodilator therapy.
Use current body weight for children.

Give early when indicated

Systemic corticosteroids are most useful when started promptly in a clinically significant asthma exacerbation, particularly when response to initial bronchodilator treatment is inadequate.

Continue asthma therapy

A short oral steroid course treats the exacerbation. Review inhaled corticosteroid therapy, reliever use, adherence, spacer technique and the precipitating factor.

Recognise severe disease

Hypoxia, severe respiratory distress, exhaustion, silent chest, poor air entry, altered consciousness or deteriorating observations require escalation.

Before prescribing

Confirm indication This calculator is designed for acute asthma only. Do not reuse this regimen for other prednisolone indications.
Adult weight The adult MOH asthma CPG describes dosing according to ideal body weight, with a maximum of 50 mg daily.
Infection Corticosteroids may mask signs of infection. Review possible bacterial, viral or other alternative diagnoses where clinically relevant.
Diabetes Systemic corticosteroids can increase blood glucose. Consider monitoring in patients with diabetes or significant hyperglycaemia risk.
Repeated courses Frequent oral corticosteroid bursts should prompt review of asthma control, adherence, inhaler technique and the need for treatment escalation.
Prolonged steroid exposure Do not apply short-course stopping advice to prolonged treatment or patients with clinically relevant recent systemic corticosteroid exposure.

What dose does the calculator use for children?

The calculator allows either 1 mg/kg/day or 2 mg/kg/day and applies the Malaysian Paediatric Protocol age-specific maximum daily dose.

What dose does it use for adults?

For adults, the available options are 0.5 mg/kg/day or 1 mg/kg/day. The adult MOH asthma CPG specifies ideal body weight and a maximum of 50 mg daily.

Does a short prednisolone course need tapering?

The adult MOH asthma CPG states that a 5–7 day course for an acute asthma exacerbation can be stopped without gradual dose reduction. Prolonged or repeated systemic corticosteroid exposure requires individual assessment.

Can I use this calculator for croup?

No. The calculator is specifically for acute asthma. Croup uses a different corticosteroid regimen.

Which syrup concentrations are included?
