Before prescribing domperidone

QT prolongation Avoid domperidone in patients with known prolongation of cardiac conduction intervals.
Electrolytes Significant electrolyte disturbances increase cardiac risk.
Cardiac disease Avoid in patients with significant underlying cardiac disease.
QT-prolonging drugs Avoid concomitant medicines that prolong the QT interval.
CYP3A4 inhibitors Potent CYP3A4 inhibitors can increase domperidone exposure. Domperidone with clarithromycin is contraindicated.
Duration Use the lowest effective dose for the shortest duration. The usual maximum treatment period should not exceed 1 week.
Lactation This calculator is not intended for off-label lactation stimulation.

In gastroenteritis, assess dehydration and prioritise oral or intravenous rehydration as clinically appropriate.

Consider infection, medication effects, pregnancy, gastrointestinal obstruction, intracranial pathology, metabolic disease and other relevant differentials.

Escalate red flags

Severe dehydration, haematemesis, bilious vomiting, peritonism, altered consciousness, shock or persistent unexplained vomiting require further assessment.

Domperidone FAQ

Can domperidone be used in a child younger than 12 years?

Domperidone should not be used in children younger than 12 years who weigh less than 35 kg. For a child younger than 12 years who weighs at least 35 kg, the Malaysian formulary lists a fixed 10 mg dose.

What dose is used at ≥12 years but below 35 kg?

The calculator uses 0.25 mg/kg per dose, given three to four times daily, with a maximum total dose of 35 mg/day.

What suspension concentration does this calculator use?

Every mL calculation on this page uses domperidone 5 mg/5 mL suspension.

How long should domperidone be prescribed?

Use the lowest effective dose for the shortest duration. The usual maximum treatment period should not exceed one week.

Why is clarithromycin important?

Clarithromycin can increase domperidone exposure through CYP3A4 inhibition and also has QT-prolonging properties. Concomitant use is contraindicated.
