Tamiflu (Oseltamivir)
Prophylaxis Dose

Post-exposure oseltamivir dosing after close contact with influenza, based on the current Malaysian Tamiflu NPRA.

✓ Post-exposure ✓ Once daily ✓ 10-day regimen

This page summarises post-exposure prophylaxis dosing from the Malaysian Tamiflu product insert for patients aged 1 year and older and within the stated weight bands. Start prophylaxis as soon as possible within 2 days of exposure.

Malaysian formulary context: the current FUKKM oseltamivir entry is treatment-only and states that it is not to be used as prophylaxis. This page therefore follows the current Malaysian NPRA-approved Tamiflu product insert for post-exposure prophylaxis.

Confirm the contextThe Malaysian indication is post-exposure prophylaxis after contact with a clinically diagnosed influenza case when influenza virus is circulating in the community.
VaccinationOseltamivir is not a substitute for influenza vaccination.
Renal impairmentDose adjustment is required in some adolescents and adults. Use the separate renal-dose reference rather than the standard table above.
FUKKM restrictionThe current KKM formulary oseltamivir entry is treatment-only and states not to use it as prophylaxis. Follow local prescribing policy and formulary restrictions where applicable.
COMMON QUESTIONS

Tamiflu prophylaxis FAQ

When should post-exposure Tamiflu prophylaxis be started?

The current Malaysian Tamiflu product insert says therapy should begin as soon as possible within 2 days of exposure to an infected individual.

Why does this page use 10 days?

The current Malaysian NPRA-approved Tamiflu product insert gives a 10-day post-exposure prevention regimen. Some international guidance, including current CDC guidance, uses a different duration for routine chemoprophylaxis. This page deliberately follows the Malaysian product insert.

What is the prophylaxis dose for a 20 kg child?

For a child aged 1–12 years weighing more than 15 kg and up to 23 kg, the Malaysian product insert gives 45 mg OD × 10 days. With 60 mg/5 mL suspension, this is 3.75 mL OD × 10 days.

What is the adult post-exposure prophylaxis dose?

The Malaysian product insert gives 75 mg OD × 10 days for adolescents aged 13–17 years and adults. With 60 mg/5 mL suspension, 75 mg is equivalent to 6.25 mL.

Why is there no dose below 1 year or below 10 kg?

The Malaysian product-insert prophylaxis table begins at ≥1 year and 10 kg. This page does not extrapolate a dose outside those locally stated limits. Other international recommendations may differ.

Can I use this table for renal impairment?

No. Renal-adjusted oseltamivir prophylaxis uses different regimens in adolescents and adults and will be handled on a separate SetelDoc renal-dose page.

Why does FUKKM say not to use oseltamivir as prophylaxis?

FUKKM describes the KKM formulary indication and restriction for oseltamivir, while the Malaysian NPRA-approved Tamiflu product insert includes post-exposure prophylaxis. This page is based on the product insert and should not override local formulary or institutional prescribing policy.
